Discovery Zone: Form for Departmental Contributors

Thank you so much joining us for our 50th anniversary celebrations and agreeing to take part in the University of Warwick's Festival of the Imagination. We want to create the best possible experience, not only for our visitors but for those contributing the content and in order to help us do this we have created a form for you to fill in. Could you please fill this in by Friday 28th August.

We require a risk assessment form for all our activities. Please fill in this Risk Assessment Form and attach it in the Health and Safety section of this from. (Further deatails can be found here: Matrix for risk evaluation)

LOGISTICS
 
Monday 12th October – Tuesday 13th October: Porters will deliver a Yorkie to your department for you to load up any equipment/material you require. (Times TBC) Wednesday 14th October – Thursday 15th October: Porters will collect filled Yorkies and other equipment/material from your department and deliver them to the Butterworth Hall before you arrive. (Times TBC) Thursday 15th October: Set up from 2pm onwards (The space will be locked overnight) Friday 16th October: On site 9.15am, open 10am – 2.45pm (School Session). Close 2.45pm-3.30pm, On site 3.45pm, open 4pm-7pm (staff, student and alumni session) Saturday 17th October: On site 9.15am, open 9.30am-7pm (open to all) 7pm Equipment/Material pack up onto Yorkies (to be left in a secure room in the arts Centre. Monday 19th October – Tuesday 20th October: Yorkies/Equipment/Materials returned to department Friday 23rd October – Empty Yorkies collected from departments
 
How many Yorkies (tall storage/transportation trolly) do you require?
 
Please estimate the total number of staff you will have working on the stand over the two days. This will help us better estimate numbers for catering and staff materials (e.g. lanyards/t-shirts etc.)
