- Authorisation required: please reply to the email acknowledgement you receive after completing the request form, copying in the person/people listed for your department and asking them to reply all with their authorisation.
- No authorisation required: you do not need to do anything beyond submitting your request, at this stage.
If your department is not listed below
Please reply to the email acknowledgement you receive after completing the request form, copying in your Personal Tutor OR Director of Undergraduate Studies OR Departmental Administrator (as seems appropriate in your home department), and ask them to reply all with their authorisation.
