This form should be completed by students who have applied to study mathematics at the University of Warwick to access support with STEP mathematics. Upon completion of this form you will be given free access to online resources that have been specifically developed for STEP preparation (this is normally setup within a week of you submitting the form). You will also be given the possibility to access a series of eight 2hr live online sessions to take place fortnightly on Saturdays beginning in February 2021. The exact dates for these sessions are to be confirmed. The sessions will be recorded for anyone who is not able to attend the sessions 'live' or so that they can be reviewed. To attend the live online session all you need is an internet enabled device. The best experience of the live sessions will be via a desktop, laptop or tablet device. It is not necessary for you to attend the live sessions and in the form below we ask whether you intend to attend the live sessions to help with our planning.
If you have any queries regarding this STEP support please contact me via the details below.
Dr Richard Lissaman

Do you expect to attend the live online sessions on Saturday mornings starting in February 2021? There is no compulsion to attend and recordings will be made available to everyone afterwards. This is just to help inform planning.
Is there anything else you think we need to know regarding your support with STEP via the resources and the live online sessions? If you intend to attend the live sessions this should include any information relevant to how you intend to access them that we may need to try to take account of.
This form will be used to manage applications for access to online resources and the live online sessions for 2020/2021 and for overall widening participation monitoring/reporting purposes. The data collected will not be used for any purposes other than those described above. NB - By submitting this form, you are confirming that the information that you have provided in support of this application is complete and true and you understand that knowingly giving false information may invalidate your application.
