Phase II Absence Form

EITHER: If your absence is related to illness
Examples of Significant Illness - Hospital admissions, operations, glandular fever, etc. Ongoing Illness - We will request the letter from the GP with your consent. This letter should explain the extent of ilness and the impact, which it is likely to have, on your studies.

OR: If your absence is related to any other circumstances. Please note that absence cannot be authorised by the Phase II Office and it is your responsibility to ensure that any time lost is made up for in discussions with your consultant, the relevant academic lead or the General Practice Office, depending on the nature of your attachment. Undertaking a planned absence will be dependent on agreeing a strategy with the Phase II Co-ordinator to ensure you gain successfully from the relevant clinical block.
