‘Best Care, Anywhere’ integrates three linked high impact primary care schemes into existing services:
(1) Increasing GP presence in the Emergency Department;
(2) GP extended hours hub offering week-day urgent appointments and weekend routine appointments;
(3) Care focussing on the older patient population with the primary care frailty team determining discharge and care planning for frail patients and managing them proactively in community based primary care settings.
The initiative covers 64 GP practices serving a patient population of 374,335. Researchers at the University of Warwick are involved with large quantitative data collection and analysis for impact evaluation.
