How to take part

Thank you for agreeing to participate in our study.

None of your data can be used without informed consent from you. This means that you are required to read and sign the consent form to show that you agree with the terms of participating and that you understand your rights as a participant.

If you choose to participate, you will need to complete the consent form below before the interview takes place, or you can request a paper version of the survey, which will be sent to you in the post with a pre-paid envelope for you to return to us.

Title of Project: Staff perspectives on factors affecting the implementation of the Picture Exchange Communication System: A qualitative analysis

1. I confirm that I have read and understand the information sheet (version 2, 27/05/20) for the above study. I have had the opportunity to consider the information, ask questions and have had these answered satisfactorily.

2. I understand that my participation is voluntary and that I am free to withdraw at any time without giving any reason, without my legal rights being affected.

3. I understand that data collected during the study, may be looked at by individuals from The University of Warwick, or from regulatory authorities, where it is relevant to my taking part in this study. I give permission for these individuals to have access to my data.

4. I agree to provide permission to the PhD researcher Andreas Paris to audio record my answers, take notes during the interview and process the data that I will provide for the purposes of this study. The interview should take approximately 45 minutes.

5. I understand that anonymised data provided by me will be written up and submitted for publication in scientific journals and presented at conferences, scientific meetings and for teaching purposes. No identifiable information about myself, my place of work, and any individuals I work with, will be shared or published. I understand that any verbatim quotations presented will be anonymised

6. I am happy for my data to be used in future research.
