Update Departmental Contact Information

 
This form should be used to report a change of departmental contact (e.g. new Director of Undergraduate Study or a new Course Co-ordinator) to Student Records Management. This will enable us to keep our records up-to-date and ensure that any communication we send is to the correct person.
