Campus Self-Isolation Accommodation Request Form 2020

If you have not been allocated a place on campus and accepted your contract online, we cannot accommodate you on campus to self-isolate. You will need to complete this form at least 72 hours before you intend to arrive on campus.

If you have requested to arrive early to self-isolate, and guidance changes and you are no longer required to self-isolate, you will need to email accommodation@warwick.ac.uk. If you have already arrived onto campus, you will need to continue to self-isolate.

Privacy notice

The University of Warwick will process your personal data on this form being your ID number, full name, email address, residence, arrival date, time of arrival, country arriving from and contact number for the purposes of arranging self-isolation accommodation, in light of the current COVID19 epidemic.

The legal basis for processing this personal data is legitimate interest and performance of a contract.

Your personal data will not be shared or disclosed to any third parties external to the University of Warwick.

Your personal data will not be transferred outside of the EEA, will be kept securely by the University of Warwick and will be retained for at least 6 months.
