If you have a mental health condition which affects your accommodation requirements then please contact the Mental Health Team regarding your accommodation needs by completing an appointment request form. Please write N/A if you are not able to answer a question on the form. Include your preferred method of communication for an appointment if you are not local to campus at the time, such as Skype or telephone and include your Skype or telephone number (including international codes) as appropriate.
You will also need to provide medical evidence written by a medical professional on official headed paper or you can ask your medical practitioner to complete the Medical Evidence form.
The evidence must include the following information or we will not be able to process it:
- Details of your mental health condition, the impact this has on your functioning and how this impacts on the accommodation that you require
- Dated within three months of the submission date
- Written in English or have an official translation with it
Any medical evidence will need to be emailed to firstname.lastname@example.org, ideally at the time of the appointment request but before an appointment is scheduled.
