Module content and teaching

Principal aims

This short course is designed to develop the skills required to manage patients with a range of chronic diseases such as diabetes, dementia, musculoskeletal conditions, depression and mental illness, and end of life care within primary care. The course will introduce you to the knowledge and skills that are needed for the delivery and organisation of high quality care, responsive to the physical, emotional, psychological and social needs of people with chronic conditions within the multidisciplinary setting.

Principal learning outcomes

By the end of the module, you will be able to: Demonstrate an advanced knowledge and understanding of specific chronic conditions in the community setting such as diabetes, depression, dementia and musculoskeletal conditions; Systematically analyse different treatments available for these specific chronic conditions and interpret these treatments in the setting of community-based care; Demonstrate awareness and explore the effects of lifestyle, culture, ethnicity, choice and psychological issues in the management of patients with specific chronic conditions in the community-based care setting; Demonstrate knowledge and understanding in the provision of palliative care for chronic conditions; Critically evaluate the role of the multi-disciplinary team in providing chronic care within the setting of community-based care; Explore and understand the impact of chronic conditions on the organisational elements within primary care and the multidisciplinary team and synthesize original ideas to plan for the care of a population with chronic care needs in the community setting.

Timetabled teaching activities

This course will run face to face for thirty hours in total, divided into five days spread over nine months. You will be taught through lectures, seminars and group work. Assessment is by 4,000 word written assignment which will be completed within the year.
