If you experience multiple, significant periods of shorter absences, that you feel are exceptionally impacting on your studies, you should speak to the Wellbeing Support Services for further advice.
To request paid sick leave of up to 28 calendar days please submit only the form below.
When your request has been processed, your funding and registration end dates will be extneded by the period of paid sick leave. You will receive a notification email to your University of Warwick email address regarding the outcome of the sick pay request and, where it has been requested, a separate email confirming your temporary withdrawal. These emails will confirm your new expected end of registration date.
You must inform your academic supervisor and/or department/DTP/CDT of any period of absence. This is in addition to completing the Sick Pay Form. Your department may arrange a meeting when you return, to ensure that you are fit for study or to see if you require any advice from Wellbeing Support Services.
Medical evidence that covers the period of absence must be included with the request. You can self-certify for the first 7 days of an absence provided that medical evidence is submitted from day 8 onwards. If the absence specifically relates to a prevuosly disclosed disability, additional medical certification is not required to be provided.
