Workshop Consent Form

I understand the information about the project that was explained to me by the researchers and given on the information sheet and project website.

I have been given the opportunity to ask questions about the project and my participation.

I voluntarily agree to participate in the project.

I understand I can stop participating any time, and I can withdraw my data from the study at any time up until 31 July 2022 without giving reasons. I understand I will not be questioned on why I have withdrawn.

The procedures regarding confidentiality (detailed in the information sheet) have been clearly explained to me.

I consent to any meetings I attend being audio recorded and/or transcribed, or notes being taken.

I understand that I can choose to stop recordings/transcription/ note-taking at any time

The use of the data in research, publications, sharing and archiving (detailed in the information sheet) has been explained to me.

I understand that other researchers in the team will have access to this data only if they agree to preserve the confidentiality of the data and if they agree to the terms I have specified in this form.

I understand that my decision to take part or not take part in this study will not in any way affect my relationship with the University, and only the researchers will have access to the information I share.

I do not want my name used in this project.

I would like my name included in the acknowledgements section of any guidance or publications that result from this project. This means I will receive recognition for contributing to the project without my specific contribution being identified or any project (i.e. no data will be connected with me or with any individuals in the project.)

Privacy notice

The data on this form relates to your consent to take part in the research project Positive Digital Practices. The date and time of your submission, your identity and the answers you have submitted will all be stored, but will not be used for any purpose other than directly relating to the Positive Digital Practices project.
