The following information is requested to help us give a timely and accurate response to your enquiry.

The University will not release personal data to you unless fully satisfied as to your identity.
 
Information sent via email will be password protected and sent as a link from http://files.warwick.ac.uk/ where you can download your disclosure.

The University does not accept any responsibility for data sent in this way.

If you wish to collect your disclosure from University house you will have to specify a date and time within the opening hours of the Information and Data Compliance team 09:00 – 17:00 – Monday to Thursday and 09:00 – 16:00 Fridays and bring a copy of your ID with you to confirm your identity.

If you are unable to collect your disclosure yourself we may be able to accept collection by another named person providing a copy of their ID via email for us to cross reference with in person on the date of collection.
 
Please provide below a clear description of the right you wish to exercise, providing as much detail as possible to help us locate the information requested. This includes dates, departments, individuals and/or any additional information which will enable us to execute your request in a timely manner. If we process a large amount of data for you we may need to ask for suitable further clarification regarding your request.

The University of Warwick will process your personal data being your title, full name, Warwick ID number, email address, job title provided in this form for the purpose processing your data subject rights request.

The legal basis for processing this personal data is legal obligation.

Your personal data will not be shared or disclosed to any third parties external to the University of Warwick.

Your personal data: will not be transferred outside of the EEA, will be kept securely by the University of Warwick and will be retained for a period of 12 months from the date the final response to your request is sent.
