Add a date field to a form

Use the date field to provide a calendar pop-up window for respondents to enter a date.
  1. Go to the form that you wish to edit.
  2. Go to Edit > Edit formsbuilder page.
  3. Under Add a text box, select Date field with calendar.
  4. In the Field name box, enter the name you want to appear as the column heading for the date field in the form submissions table. For example:

    Start date
  5. In the Label box, type the question or instructions to show the respondent on the form – for example:

    Choose a start date

    If you don't specify a Label, the Field name displays next to the calendar pop-up instead.

  6. Choose a Width of the date box as a percentage of the page width. For example, 100% spans the entire page.
  7. Optionally, in the Prefilled text box, enter a hint or example answer to help the respondent understand the information you're asking for. This text appears in the text box when the form loads.
  8. Choose whether to display the Label above the text box or at the left. The Label displays at the left by default.
  9. To show the date field only to editors of submissions – and hide it from respondents – select Only visible to editors.
  10. To make the date field mandatory – the respondent must choose a date before they can submit the form – select the Required checkbox.
  11. Select Save.
