Report Form

Submit this form only if you have already received funding from the Club Development Fund and wish to provide feedback for how the funding has helped your club. This report is mandatory for all successful recipients of the Club Development Fund. Reports must be submitted one month after the project or development end date. At the latest this should be 30th June, 2019.

The data on this form will be used to compile a report on how your Club Development Funding was used. By submitting this report you are agreeing that any photos and text may be used in our publications and on our website. The data is for the use of Warwick Sport and Warwick SU and will not be disclosed to external organisations, other than those acting as agents for the University. Your data will be will be shared within the University, as deemed appropriate. Your data is also subject to the University’s Website Terms and Conditions and the central University Data Protection Policy.
