If you are returning paperwork from participants who have withdrawn or been discharged to me by post (i.e. their treatment booklet where you record all of the exercises that they have been doing), please photocopy the paperwork with the participant’s name removed from the front page (I use a post it note to cover the participant details then photocopy it) and send the copy to me. The originals should be kept and filed/archived as you would with normal patient records.

We do not need a copy of the referral form or any other forms at this time.

Treatment withdrawal forms should be faxed to the number on top of the form if possible, unless you don’t have access to a fax machine, then they can be posted to me.
