This may depend on each individual GP practice and their processes for making appointments. If possible, identify a nominated individual from each practice to either send or take the MFFP forms. They can then arrange appointments for the patients and pass the forms onto the GP(s). We also need a process to ensure return of the completed form to the PreFIT study office.
