How to create a Form page
Hover over Edit and click on Create new page. By clicking on the cross in the top right hand corner, change the page template to a Form template. Provide a brief, unique URL.
Under Additional options, decide whether you want the form to span the whole page, or the centre only, by unchecking the box. Publish this page. You will now have a blank Formsbuilder page.
How to include information about the form
Select Edit surrounding content from the quick edit menu. You can add any text, photos or multimedia here, eg why the from needs completing. Do not delete the Content placeholder or you text won't appear!
Upload this content and Publish.
How to format the form
On the published page, below the default privacy statement which is automatically displayed, select Edit this form.
From the choices menu on the left, add a Personal details field, which will autofill for the logged in user. Delete any field which is not required.
Use the X button to delete an option, the ^ arrow keys to move the field up/down the sequence, or the pencil icon to edit the field options.
Publish the page.
Note: A Form page cannot be changed to a different template so be sure that this is the type of page you want to work with.
Before you design the form:
1. Think carefully about what information you want to know and what you want to do with that information?
2. Familiarise yourself with the type of form element options (see next page) before you begin to write.
1. Create a new Form page and add two or three basic form elements.
2. Edit the surrounding content and add some appropriate text.
To make text bold in a label, insert an * at either end of the text.
The Privacy statement is a compulsory element. Edit the Privacy statement to make it more appropriate for your users.
