If you normally work in the GP practice, photocopy the MFFP forms and leave one copy in their GP records and leave the original version in the red PreFIT box file. If you do not normally work in the GP practice you will need to photocopy the forms and leave one copy with the GP practice and keep another copy for your clinical records and put one copy in the red PreFIT box file. Your local Research Nurse or the Study office will arrange to collect the completed forms from the practice. For more remote practices, we may ask the Practice Manager to return forms to the PreFIT office.
