The PreFIT treatment database is an Excel spreadsheet that will be emailed to you each month to ask for an update on the patients referred for treatment. This is just so we can keep up to date with how things are going, as there is sometimes a lag between getting all the paperwork back. This would be really helpful for us. You may be very familiar with MS Excel databases, but if not, don’t worry – just phone or email and we can go through it. The spreadsheet has 2 ‘sheets’ which you can view by clicking on the tabs at the bottom of the page: 1. The Summary Sheet and 2. The Referrals sheet. *The Summary Sheet* • This first Summary sheet lists the number of referrals to be seen and give you their risk level (high or intermediate). As you complete the next sheet, this information will update automatically so you do not have to enter anything here. *The Referrals Sheet* • This sheet has a column header and then all the participant ID numbers are listed down the left hand side of the sheet • The 3rd column (Column C) is the participants’ risk status e.g. high or intermediate 1, 2 or3 (the key for this is on the summary sheet). • The 4th column (Column D) is for the date of their falls assessment, therefore please enter the date that you assess each corresponding patient in that column. • If a patient dies, please enter the date that you were made aware of the death in the 5th column (Column E) - this information is more likely to come to the university first so may be filled in in the office. You can then add in the Comments box at the end that the patient died (Column O). • If the patient wishes to withdraw from treatment before they come for assessment then please record the date that they told you of this decision in ‘patients withdrawn from treatment prior to assessment’ (Column G) and add any comments. • If a patient wishes to withdraw from treatment after they have been assessed, then please enter the date that they informed you of this in the ‘patients withdrawn from treatment after assessment’ (Column H) and add any comments. • If you make any referrals on to either the GP, Physio or any other service, please indicate this with Y/N in the appropriate column (Columns I to N). And if you know the date of their assessment with any other health professional then please complete this also in the appropriate column. • The comments column is for any other information that you feel is necessary or important for us.
