Add a single-choice ratings set
Use a single-choice rating set to enable respondents to rate a list of items against a numerical or descriptive scale. In the example below, we ask respondents to rate their satisfaction with aspects of a conference:

Browse to the form you want to edit.

Go to Edit > Edit formsbuilder page:
Alternatively, select the Edit this form button at the top of the form:

Under Allow the user to make a choice, select Single-choice rating set:

You see the Add/Edit a set of check-boxes screen:

In the Field name box, enter the name you want to appear as the column heading in the form submissions table. For example:
Satisfaction

In the Label box, enter the question or instructions to show the respondent on the form – for example:
Rate your satisfaction with the following:
If you don't specify a Label, the Field name displays on the form instead.

Enter your scale in the Possible ratings section. You can use numbers (1-5, for example) or words. For the conference example above, we ask the respondent to choose one of five ratings:

Very satisfied

Satisfied
-
Neither satisfied nor unsatisfied

Unsatisfied

Very unsatisfied
To add additional rating options, select Add new item. To remove a rating, select the Click to remove button.

In the Items to rate section, enter the first item you're asking respondents to rate. To add additional rating items, select Add new item. To remove an item, select the Click to remove button.

If you want one of the ratings to be preselected when the form loads, use the Default rating drop-down.
Tip: When you ask for a rating, we recommend you leave the Default rating blank. Otherwise, the respondend might skip or miss the question and submit the form with the default rating selected.

To make the question mandatory – meaning the respondent must select ratings before they can submit the form – select Required.

To show the rating set only to editors of submissions – and hide it from respondents – select Only visible to editors.

When you're finished setting up the question, select Save to return to the Edit form screen.
