PET-CT Scan Posting Notification Form

Scanning Centres or participating sites (which ever is applicable) should use the PET-CT Scan Posting Notification Form once a PET-CT scan has been posted to the Core Lab at the Paul Strickland Scanning Centre in order to notify the Trial Co-ordinator that the scan has been sent. Please complete a separate form for each patient.
