Sexual Health and History Survey

Part One: Information and Consent

As part of the events of SHAW, we would like to assess sexual health awareness and practices of healthy adults aged 18 years and older. In this questionnaire we will ask you questions about your sexual experiences. The survey is completely confidential and anonymous. Only the researchers will see participants’ responses and all responses will be summarized in group form.
