Midlands Graduate School ESRC DTP Equality and Diversity Data Form

As part of our reporting requirement to the ESRC, Doctoral Training Partnerships are required to provide diversity statistics for the overall funding applicant pool. In order to comply with our reporting requirements, we would be most grateful if applicants who have applied for DTP funding from the Midlands Graduate School could complete the following information.

We are requesting that applicants provide their name as we are required to supply overall diversity statistics for awarded applicants. The Midlands Graduate School needs to be able to extract this data and therefore we require to know applicants names so that the relevant data can be gathered, should you be successfully awarded funding.

The data collected in this form will used for the purpose of equal opportunities monitoring and will be shared with the ESRC in an anonymised manner to this end. We will not use the data for any other purpose.

The University of Warwick will use your personal data provided in this Equality and Diversity data form for the purpose of fulfilling its reporting requirements to ESRC. Your personal data will not be shared or disclosed to any third parties external to the University of Warwick except with: administrators and academics across the 6 institutions that form part of the Midlands Graduate School ESRC DTP (Universities of: Aston, Birmingham, Leicester, Loughborough and Nottingham) who will only receive the data in an anonymised format and will use it for the purpose of equal opportunity monitoring; and the ESRC who will only receive the data in an anonymised format and will use it for the purpose of equal opportunity monitoring. By submitting this form you understand that your personal data will be collected by the University of Warwick and shared on an anonymised basis with the ESRC for the purposes outlined above and the personal data will be kept for the lifetime of the Midlands Graduate School ESRC Doctoral Training Partnership.
