PET-CT Receipt of Scan Notification Form

The Paul Strickland Scanner Centre should use the Paul Strickland PET-CT Receipt of Scan Form to notify the Trial Co-ordinator that a PET-CT scan has been received from a PET-CT scanning centre or participating site. Please complete a separate form for each patient.

Paul Strickland PET-CT Response Form

The Paul Strickland Scanner Centre should complete and submit the PET-CT Response form to inform the Trial Co-ordinator of the results of the 2^nd reading of scans. Please complete a separate form for each patient's scan.
