Study Questionnaires

How to complete study questionnaires?

Complete Questionnaires via Post

If you do not have access to the internet or a smartphone or would prefer to physically fill these out you will be sent a postal questionnaire.

  1. Complete all the questions in the questionnaire booklet, as best as you can
  2. Once complete, return questionnaire in the freepost envelope enclosed
  3. If you would prefer to complete these over the phone please give us a call on 024 7615 1763

Completing Questionnaires via a Text Link

If you have access to the internet and a smartphone, you will be sent a link via text message.

1. A questionnaire link will be sent to you via text, an example of this is below

 

2. Follow the link. You will be taken to the iRehab database (pop out box) and prompted for a PIN.

3. Once the pop out box appears, a PIN will be sent to you via text

4. Enter the PIN sent to you via text and this will unlock your questionnaires.

Completing Questionnaires via an Email Link

If you have access to the internet, have a phone but not a smartphone, you will be sent a link via text email.

  1. A questionnaire link will be sent to you via email, an example of this is below

2. Follow the link. You will be taken to the iRehab database (pop out box) and prompted for a PIN.

3. Once the pop out box appears, a PIN will be sent to you via text

4. Enter the PIN sent to you via text and this will unlock your questionnaires.
