Add a date field to a form

Use the date field to provide a calendar pop-up window for respondents to enter a date.

  1. Browse to the form you want to edit.

  2. Go to Edit > Edit formsbuilder page:

    Alternatively, select the Edit this form button at the top of the form:

  3. Under Add a text box, select Date field with calendar:

  4. You see the Add/Edit a date field screen:

  5. In the Field name box, enter the name you want to appear as the column heading for the date field in the form submissions table. For example:

    Start date
  6. In the Label box, type the queston or instructions to display on the form - for example:

    Choose a start date

    If you don't specify a Label, the Field name is displayed instead.

  7. Use the Width drop-down list to specify what percentage of the available page width the field should occupy.

  8. Optionally, in the Prefilled text box, enter a hint or example answer to help the respondent understand the information you're asking for. This text appears in the text box when the form loads.

  9. By default, he Label displays to the left of the date field. If you'd prefer to have it appear above, select the Label on top checkbox.

  10. To hide the date field from respondents, select the Only visible to editors checkbox. If this is enabled, only users with permissions to edit submissions will see the date field.

  11. To make the field mandatory - the respondent must choose a date before they can submit the form - select the Required checkbox.

  12. Select Save to return to the Edit form screen.
