File attachments to forms

Use the file attachment field to enable respondents to upload and submit files. While there is no maximum file size limit, it's possible that files over 5 GB in size may not upload successfully.

Add a file attachment field

  1. Browse to the form you want to edit.

  2. Go to Edit > Edit formsbuilder page:

    Alternatively, select the Edit this form button at the top of the form:

  3. Under Add a text box, select File attachment field:

  4. You see the Add/Edit a file attachment link screen:

  5. In the Field name box, enter the name you want to appear as the column heading in the form submissions table. For example:

  6. In the Label box, enter the question or instructions to show the respondent on the form – for example:

    If you don't specify a Label, the Field name displays on the form instead.

  7. Specify the number of files respondents can attach using the Maximum attachments box.

  8. When the Accept file types box is empty, respondents can attach files with any file name extension. To restrict the types of files, enter the accepted extensions separated by commas - for example:

    doc, docx, pdf, txt, rtf, odt, ott, oth, odm

    Tip: When you restrict file types, we recommend that you list the types you accept in the Label box, so respondents know of any restrictions before they try to upload a file.

  9. By default, the Label displays to the left of the text box. If you'd like to display the Label on top, select that checkbox.

  10. To make the attachment field mandatory – meaning the respondent must attach one or more files before they can submit the form – select Required.

  11. To display the attachment field only to editors of submissions – and hide it from respondents – select Only visible to editors.

  12. Optionally, to help identify submitted attachments, you can Prefix file name with one of the following options:

    • The respondent's University ID number

    • Their Username

    • Their Department

    • The submission Date(ddmmyy)

    • Anything you enter in the This text box.

  13. When you're finished editing the question, select Save to return to the Edit form screen.

Include file attachments with submission notifications

Note: Email is not a secure medium. By default, submission notifications do not include the submission data or attachments – notifications contain a link to view the submission data in SiteBuilder. Respondents' files are available to download from the form submissions table. To include respondents' files with submission notification emails, enable this setting in the form properties.

  1. Browse to the form you want to edit.

  2. Go to Edit > Edit formsbuilder page:

  3. Under Modify the form, select Form properties:

  4. The Edit your form properties page appears:

  5. Under How should we tell you about completed forms?, select the checkbox to Include submission data in email.

  6. In the warning pop-up, select Yes.

  7. Select the Include files attached to the form checkbox.

  8. Select Publish to return to the Edit form screen.
