Please note: your request will be automatically deleted if you do not attach the confirmation from your department. If you have received confirmation via email, please save/print the email as a .pdf document and attach it with this form. You must attach: Confirmation in writing (letter or email) from your academic department. It must contain the following information:

- the official original project/dissertation submission deadline for your course, and

- the date you completed all the requirements of the course (i.e., you have sat all exams and submitted all work); there should be nothing pending.
