Pre-University Summer School Pre-Arrival Form

Students should complete this form with a parent/guardian to ensure we have all the information available. You will only be able to attend the Pre-University Summer School if you complete all of our information requirements.

Contact Details

It is important we have direct contact details for students on our programme, so that we can communicate with them effectively on arrival in the UK and throughout the programme. Please provide contact details below. 

Please ensure that your child/ward has access to a mobile phone that they can use in the UK. Students will be able to access free Wi-Fi in their accommodation and in university buildings.

Emergency Contact Details

Please provide the details for people who we can contact in the case of an emergency. These contacts must be available at all times during the programme.

Please note, in an emergency we will try to make contact in the order these details are given on the form. If possible, at least one of these contacts should be UK based.

Medical, Access and Dietary Requirements

It is important that we know of any medical information, including any disabilities, medical conditions or allergies so that we are aware of these, and if necessary can make provisions during your stay. We may require a meeting in advance with the student and parent and guardian and will contact you if this in the case.

If the student has no additional needs or medical information to share, please indicate "not applicable". 

Accommodation Preference

During the Pre-University Summer School, students stay in our Sherbourne Halls of Residence. Students are allocated a single study bedroom with private en-suite bathroom. Each flat has a shared communal space. All students agree to abide by our Code of Conduct and not enter each other's bedrooms or any flats other than your own.

Our standard process is to allocate students to single sex flats. However we are also able to offer mixed sex flats for a limited number of students aged over 16. Please use the drop down box to indicate if you would like to be considered for a mixed sex flat. Places in mixed sex flats will be allocated at the discretion of the Summer School team. Your parent/guardian will be given our contact details and can request to be informed of your preference. We recommend talking to your parent/guardian if you wish to select a mixed sex flat if possible.

ID Card Photo Upload

Please provide a recent headshot photograph of the student in .jpeg, .png or .gif format. The photo should be of good quality, and not blurred and against a white or very light background and be at least 240px high x 240px wide. The student should be looking straight ahead and their face and eyes must be clearly visible, if they wear glasses ensure there's no reflection and the frames don't obstruct their eyes. Caps, hats and dark glasses should not be worn in the photograph. Religious headwear is allowed but it must not cover a student's face.

The picture can be taken on a smart phone, and does not need to be a professional photograph.

Travel Information - Joining and Departing the Programme

Arrivals

If you require an airport pick up at the start of the programme, please provide us with your flight details below.

Arriving Independently

If you are making your own way to campus on 18th July, please provide details of when you will arrive on campus and how you will get there.

Departures 

If you require an airport drop off at the end of the programme, please provide us with your flight details below.

Departing Independently

If you do not require an airport drop off on 28th July, please tell us what onward travel arrangements you are making to depart from the programme. 

Privacy notice

The information collected via this form will be used to facilitate the planning and delivery of the Pre-University Summer School. The information provided will be recorded and associated with the student's name and record, and the data will not be used for any purpose other than stated. 

We will keep the data for no more than 3 months after the end of the Pre-University Summer School programme.
