The following form should be filled out by the applicant (or lead applicant for groups) as named on the initial application form. Please do consider helping us promote the Fund in future by allowing us to use details about your project in publicity. We hope that you find the assistance that the Fund has provided valuable and you will help us to promote the wide range of activities that have been undertaken as part of it.

In order to promote the Lord Rootes Memorial Fund and to encourage other students to apply, from time to time we would very much like to use your name, department and project title in publicity material.

I acknowledge that my completed report will be stored in the Modern Records Centre following the successful completion of my project and it will be available to be viewed by any user of the centre.

I consent to the inclusion of my name, department and project title in publicity material connected to the Lord Rootes Fund (Please untick this box if you do not wish your details to be used)
