I confirm that I have read and understand the information sheet (version 3, 08/03/22) for the above study

I have had the opportunity to consider the information, ask questions and have had these answered satisfactorily

I understand that my participation is voluntary and that I am free to withdraw my participation at any time without giving any reason

I understand that my contact details will be used for the sole purpose of communicating details of the workshop series, final event and completing the questionnaire

I understand that my contact details will not be shared with any third party and be stored for 6 months after which they will be deleted

I understand that data collected during the workshops may be looked at by individuals from The University of Warwick. I give permission for these individuals to have access to my data.

I give my consent for any poems I submit at the end of the workshop series and any audio/video recordings of my poems to be published on the project website (for 5 years, unless consent withdrawn)

I give my consent to the use of (anonymised) data from the questionnaire for this study and understand that I will not be able to withdraw this data after 3 months of returning the questionnaire

I am happy for my data to be used in future research

I agree to take part in the above study
