Self-Certification (medical)

You can self-certificate to say that you are ill. Self-certification is where you take responsibility for your own diagnosis and inform the HetSys admin team who will in turn inform your supervisor. It is a similar system to that which runs in the workplace. Self-Certification is only allowable for up to 7 days of illness; if your illness is longer than 7 days, then you must provide a medical note from a recognised medical practitioner

The data on this form relates to your self certification of illness. The date and time of your submission, your identity and the information you have provided will all be stored, but will not be used for any purpose other than administering and recording your attendance. If there is an issue with allocated teaching duties, we would like to reassure you that the information that you have provided is treated with utmost confidence and is viewable only by Sarah Jarratt or a designated HetSys contact, the only data that is shared in such cases is your name and the period of absence in date format.
