The aims of the project were to:

Develop service user-led recommendations to enable smooth transition for people with memory loss from hospital care to the community.

Explore the experiences of carers and people with memory loss of service provision for 12 weeks after hospital discharge particularly what works well and what can be improved.

Assess the enablers and barriers to providing good discharge planning by health and social care professionals, including the availability and uptake of services.

Ascertain the involvement of carers and people with memory loss in decision-making around service provision at and after hospital discharge.
