Pre-University Parent/Guardian Consent Form

Students participating in the University of Warwick’s Pre-University Summer School programme are required to maintain the standard of behaviour set out in the Student Code of Conduct.

Student participation in the University of Warwick’s Pre-University Summer School programme requires consent from the parent or guardian.

By paying the programme fee the parent/guardian implicitly agrees to the arrangements for the programme contained within the Parent/Guardian Information.

We also ask that the parent/guardian signs the declaration below to provide explicit consent, and to acknowledge they have read the information provided.

Declaration by parent/guardian

I acknowledge that by allowing my child to attend the Pre-University Summer School, I am agreeing to the following:

I agree that the named student may attend their Pre-University Summer School programme and all associated activities, hosted by the University of Warwick.
I have read the Parent/Guardian information and consent page and understand that there will be times during the programme where students will be unsupervised.
I confirm that my child/ward can swim to a suitable level for undertaking water based activities and if not that they will wear a life vest when required.
I give the staff of the Summer Programmes team the permission to seek appropriate and timely medical care for my child or ward named below in case of emergency while in attendance at the event.
I have read the Student Code of Conduct Agreement, and discussed it with my child/ward and agree for the named student to be removed from the programme if they are found to be in breach of the rules and regulations of the Programme and will make arrangements to remove them from the programme when I am contacted.

Privacy notice

The information collected via this form will be used to facilitate the planning and delivery of the Pre-University Summer School. The information provided will be recorded and associated with the student's name and record, and the data will not be used for any purpose other than stated.

We will keep the data for no more than 3 months after the end of the Pre-University Summer School programme.
