This form is to request permission to submit your dissertation, despite the departments recommendation that you delay this to the next submission period because you have three or more resits.

Do not fill in this form if you have two resits or less.

Before filling in this form please make sure that you have an email from your dissertation supervisor and personal academic tutor. You will not be able to submit the form without this.
