Please do not use this form if you have received a Tier 4 visa refusal.

There is a separate form dedicated to Tier 4 visa refusals.

If you received your CAS from the Admissions Team and have made your Tier 4 visa application in the UK independently, and you have used 'International Student Office, University of Warwick' as your correspondence address, please use the 'Independent Application Notification Form' and attach required documents.
