Football League referees
Please fill out this form after refereeing matches in any of the Warwick Sport football leagues.
- You will be given the option to rate the behaviour of each team on a scale of 1-10, with 1 being the worst and 10 being the best.
- There will also be an open text box where you can make comments about the team or any specific player’s conduct.
- Any incidents reported through this form will be followed up on by Warwick Sport, and where necessary action will be taken against teams and individuals.
- You are also able to use the open text box to make positive comments about particular teams or players, should you have a pleasant experience that you would like to report back on.
