Please complete this application after discussion with your mentor. You should complete the questions firstly in a separate Word document.

I confirm my topic is not highly sensitive and will not involve people taking part in illegal activities or other activities that represent a threat to themselves or others (e.g. sexual activity/ criminal acts/ drug use or professional misconduct) nor will I encourage participants to discuss these topics. 15. I confirm I will give my participants a participant information leaflet and consent form to complete before taking part in my study (required)

17. Note: Interview participants must be able to withdraw at any time. Participants of a questionnaire cannot withdraw after they have submitted their form as their entry should be anonymous.

The Widening Participation Ethics Committee will need to review all documents which will be given to participants in your study. There may be more than one type of each document for each study i.e. multiple participant information sheets if there are different participant groups.

I understand that Widening Participation Ethics Committee grants ethical approval for projects, and that it is my responsibility to get approval and permission before starting the project.

I confirm I am familiar with and will conduct my project in line with the General Data Protection Regulation (GDPR) and Data Protection Act 2018 (DPA 2018), reporting any data breaches to the University’s Information and Data Director: DPO@warwick.ac.uk.

I understand that I must not begin research and related projects with human participants, their data or tissue until I have received full approval from Widening Participation Ethics Committee.

I understand that any changes that I would like to make to this study after receiving approval from Widening Participation Ethics Committee, require further review. As such they must be resubmitted to the committee via the online portal before such changes are implemented.

 

Privacy notice

The data on this form will be used as part of your application for research ethics. The date and time of your submission, and your identity (where submitted) will also be stored, but will not be used for any purpose other than the ethical approval process.
