Warwick University - Publishing My Work - Consent Form


Well done for taking part in our activity!

We hope you enjoyed it and we are very proud of the work you produced. We would like to share this work with the wider community (our staff, schools, other universities and the local community). 

We always ensure that we only publish work that respect your privacy and in line with our Safeguarding Policy. 

After giving your consent to use your work for marketing and communications purposes, you can still decide to withdraw your work at any point from our website. To do so, you can send an email to outreach@warwick.ac.uk stating your name and with the link to where your work is on our website. We will then remove it. Please note that if your work has already been shared, we might not be able to remove it from our printed marketing material or on other websites that do not belong to the university.



 

BEFORE GIVING YOUR CONSENT FOR US TO USE YOUR WORK ON OUR WEBSITE AND SHARE IT WITH OUR SCHOOLS AND COMMUNITY PLEASE CONSIDER THE FOLLOWING: 

 

- make sure that you are happy to share the content of you work with the wider public before you give us your consent to do so

- make sure that your work protects your privacy and the privacy of others. Your work should not display any contact details or personal details such as addresses, school, full name

- we will need your parents/carers consent to use your work 

- we will not publish any work that is not in line with the University values of respect and equality or compromise the safety of anyone, especially children

- the university reserves the right to remove any work if deemed in breach of these values or compromising the safety of anyone 

- all work will be published on our website for the public to see and may be used in our printed material to showcase the activity you took part in.

- we will only use the email address and phone number that you provide in this form to contact you in case we have any questions relating to your work and will not retain details if your work is removed from our website.

- By signing this form you give us consent to use the material uploaded in the way and for the purpose described above and to contact your parent/carer to gather their consent.
