This Form is provided as generic guidance to Engineers, Officers and Management teams so as to assist discharge of legal responsibilities. The safety of persons working with plant and equipment should be taken into account at all times as well as the environment in which it is situated.

 Prior to carrying out tasks Risk Assessments must be produced and where identified 'Method Statements' for safe working must also be produced. Due to the nature of some tasks the activity may require a 'Permit to Work' as well as a 'Method Statement' and local inductions into specific hazardous areas. This will ensure a safe system of working will be adopted prior to work commencing.

 Note: This form shall be submitted for duration's not exceeding 5 working days max during the week or 2 day max for the weekend. Forms shall be submitted on a weekly basis regardless of duration of activity
