Thank you for attending the latest event from the Professional Services Excellence Network. Complete the form below to confirm your attendance for our records, we'll use this data to review how popular events are, what drop-out rate to expect and which events are appealing to staff from various departments.

You'll need to complete the form for every event you attend and should be invited to do so once you arrive.

Please note that this form is intended to be completed on the day of the event, and not to be used ahead of an event to confirm your place. We'll also invite you to complete our feedback form after the event, and you can find this here. 

Privacy notice

This form will be used to manage attendance at events as part of our Professional Services Network. Your attendance at the event will be recorded and associated with your name, and we may email you to request feedback, but no other personally identifiable data will be stored, and the data will not be used for any purpose other than as a record of the event.
