Unusual options form for Physics students

Please make sure you have read the guidance in the Course Handbook before completing, to avoid disappointment. There is no need to fill out this form for language modules or WBS modules. For WBS modules remember that you must also register on my.wbs as well as eMR.

You must attach an email from each of the module leader and your personal tutor, indicating their approval, otherwise your application may be rejected.
