Monitoring Point Absence Form

This form allows students to notify the support office of any absences regarding a monitoring point. This must be received at least three hours before a class or monitoring point event starts unless there is an issue that would not have been possible to forsee. This allows the support office time to notify the class tutor and to process the submission. Any submissions received after this time will result in an unauthorised if there is no valid reason.

Monitoring point 1 and monitoring point 2 ONLY can be authorised after the event to give you time to become familiar with the system.

All submissions will be reviewed & must be in line with university regulations to be approved as missed authorised, ie. due to sickness, rather then visting a friend.

If you click the attendance tab on Tabula it will list all monitoring points throughout an academic year.

If you believe you will need an extension or waiver for a piece of work this is not the correct form to fill out. Instead, you must submit a mitigating circumstances form via the personal circumstances tab on tabula.

This form will be used to manage absence at an event. Your absence at the event will be recorded and associated with your name, but no other personally identifiable data will be stored, and the data will not be used for any purpose other than as a record of the event.
