SHARP: the start of a rehab journey in a hyper acute setting

Will be presenting the SHARP project (Specialist Hyper Acute Rehabilitation Pathway). The lecture will include data about patient length of stay in 2 phases: rehabilitation – transfer ready; transfer ready – date of discharge. They also collect UKROC data about severity of injury, rehab prescription and actual vs. proposed discharge destination. The presentation of this project would be of interest to the network and brings a practice example of how this trust has invested in rehabilitation in an acute setting.
