Placement Provider Feedback Form

Thank you for the support you have provided by supplying a year in industry placement to a University of Warwick, Life Sciences student.

The support you provide is invaluable to the development of the student’s employability skills. The time the student spends within your department has helped them hone their skills, gain valuable insight and experience in your organisation and sector, and helped determine their choices for their career in the future. We also hope that the experience was valuable to you too.

Your feedback is a vital part of our process to help us improve our industry placement offer. We are interested in hearing about your needs, what worked well and what could be improved. We would be grateful if you could take some time to complete the questionnaire below.

If you hosted more than one University of Warwick Life Sciences student, please fill in a separate form for each student.

Placement Presentations:

In October there will be a poster presentation evening where placement students will display, through a poster, everything they have learned from their placement with you. If you feel your placement student deserves to be recognised and receive a prize for ‘best placement student’ at this event, please include their name and a brief supporting sentence below.
