CHAT-P

CHAT-P is an online prostate cancer specific holistic needs assessment. It has been designed to be used as both a tool to help men identify any concerns to a member of their healthcare team, and also to support patient self-management through embedded links to further information. The system, implemented with InfoFlex, is adaptive, allowing men to select areas of concern from 11 top level domains, ranging from physical symptoms to financial issues. Their selection opens up a series of menus containing more detailed items, enabling men to quickly identify and communicate specific concerns to their health care professionals to assist effective care planning. The results of CHAT-P and embedded care plan can easily be added to the patient’s records and shared with other members of the healthcare team.

After several rounds of user testing and refinement across a five year period, CHAT-P is now ready for implementation within the clinical care pathway.

CHAT-P can be used by patients at any stage of the care pathway, from post diagnosis onwards. It is also suitable to be used in both primary and secondary care.
