A new user assessment request form

Before you submit this form you have to

1) Request equipment training, including submission of user’s samples’, COSHH and project risk assessments and local exhaust ventilation system user training certificate, if you are going to use the fume cupboard, via https://warwick.ac.uk/fac/sci/physics/research/condensedmatt/silicon/facilities/requestform/.

2) Attend the induction lecture and/or refer to its slides on AMF Teams.

3) Learn how to use the equipment by reading its quick start guide on AMF Teams.

4) Arrange a clean room and equipment training session with Alan Burton. It occurs on Monday.

5) Attend and record, in the equipment’s usage log (Excel file) on AMF Teams, at least 1 hands on equipment training session with Alan Burton.

6) Demonstrate to Alan Burton your knowledge and independent usage of the equipment.

7) When you are ready to become an approved user then submit the “A new user assessment form” via https://warwick.ac.uk/fac/sci/physics/research/condensedmatt/silicon/facilities/microfabrication/nua. Assessment occurs on Wednesday morning.

8) As soon as you become an approved user then access to the clean rooms and equipment booking system is granted via Alan Burton or Maksym Myronov, if Alan is absent. Start using the equipment independently.
