Contextual Admissions Declaration Form

About this form

Warwick Medical School is committed to fair access to medicine. Contextual admissions are used to recognise that applicants’ educational achievement and opportunities may have been affected by personal, social, financial or educational disadvantage. This helps us consider achievement and potential in a broader and fairer context.

Please complete this form if you wish to be considered for contextual admissions. If you are found to be eligible, a contextual flag may be applied to your application in line with Warwick Medical School’s approved contextual admissions process, *link to webpage*. Contextual status does not guarantee an interview or offer. All applicants must still meet the published minimum academic requirements and any relevant admissions thresholds.

Contextual admissions considerations are only available to applicants who have UK Resident Home Fee Status.

To be eligible for contextual admissions, you must normally meet at least one major indicator. Additional contextual indicators may be taken into account to help Warwick Medical School understand the full context of an application and for monitoring purposes. We ask you to indicate all relevant indicators so that the admissions team can understand the full context of your application and support monitoring and evaluation of the process.

Deadline for submission: 31st October 2026 (anything received after this date will not be considered)

Major Indicators

Please tick all indicators that apply to you and provide any relevant details. To be eligible for contextual admissions, applicants must normally meet at least one major indicator.

Care experience: For the purposes of contextual admissions, this refers to having spent time in care at any stage of life, including foster care, residential care, looked-after status at home, or kinship care.
Estrangement: For the purposes of contextual admissions, this refers to being irreconcilably estranged from both parents, or from your sole surviving parent, such that you have had no communicative relationship with them for a significant period and this is unlikely to change.
Refugee or asylum background with home-fee eligibility: This may include refugee status, humanitarian protection, asylum seeker status, or another relevant protected status, where this aligns with the Home fee eligibility requirements for the policy.
FSM-6 / sustained free school meals eligibility: For the purposes of contextual admissions, this refers to applicants who have been eligible for free school meals during their school years in a way that meets the agreed WMS definition and evidence requirements

Additional contextual indicators

UCAT bursary eligibility: Please tick this box if you were eligible for the UCAT bursary. UCAT bursary eligibility is treated as a verified indicator of low-income disadvantage, but it will not normally be counted separately where the same underlying disadvantage is already being recognised through FSM-6.
First generation in higher education: Please tick this box if, to the best of your knowledge, neither of your parents or guardians has completed a higher education qualification.
Hold significant caring responsibilities: Please tick this box if you have had significant caring responsibilities, for example as a young adult carer, the main carer for a child, or the main carer for a dependent adult.
