Risk Assessment
The Risk & COSHH Assessment forms are designed to assist management units that comply with relevant items of risk assessment - based on health and safety legislation.
Before creating a risk and COSHH assessment ensure you complete the risk assessment course.
For risk and COSHH assessments, download the required form below then;
1) Save an electronic copy of the risk assessment to the local building H&S folder for the space you are working in. If this does not exist then save a copy to your hard drive.
2) Have the risk assessment reviewed and countersigned by the line manager, person in charge or a competent person, making any changes requested by the counter-signee.
3) Print and sign a paper copy of the assessment. For lab/workshop spaces place in provided red folders. All users of the space must read and countersign the assessments to acknowledge the hazards and controls.
All risk and COSHH assessments MUST be reviewed every 12 months, when there is a significant change that affects the assessment or when an incident has occurred.
Safe System of Work (SSOW)
The SSOW covers process, associated equipment and the required safe methods and may well require information that has already been used to complete both the RA and COSHH assessments. For more information SSOW assessments, click here. To search through previous SSOW assement forms, click here.
